Anyone — a beneficiary, partner, supplier, staff member, volunteer or member of the public — can report a concern to Phoenix. This page explains how reporting works and how we protect you; our internal policies set out the full procedures for our staff, partners, suppliers and contractors.
1. What you can report
- The quality of a service or distribution, or the conduct of a staff member, volunteer or supplier.
- Safeguarding concerns: any actual or suspected sexual exploitation, abuse or harassment, especially involving children or vulnerable people.
- Suspected fraud, bribery, corruption or misuse of resources.
- A conflict of interest, or any concern about how your personal data is handled.
2. You do not need proof
Report as soon as you have a reasonable concern or suspicion — you do not need complete evidence or certainty. If in doubt, report; it is our responsibility to assess what you tell us and decide what happens next.
3. Confidentiality and no retaliation
- You may report by name or anonymously.
- Every report is handled with strict confidentiality; only people who need to know are told, and only what they need to know.
- We do not tolerate any form of retaliation, threat, intimidation or adverse treatment against anyone who reports a concern in good faith, even if the investigation does not confirm it.
- Reporting a concern never affects anyone’s access to our assistance.
4. What happens after you report
- You receive a reference number as soon as you submit your report.
- A designated, impartial person or committee reviews it confidentially.
- Safeguarding, fraud and corruption reports are treated as urgent and seen only by senior management.
- We take the action the situation requires, in line with our policies and, where relevant, the law.
You can submit your report directly through our complaints and feedback system below.
